Referral Process

Whether you're a HCP provider or a HCP client, referring to AgeHappy for physiotherapy support is a simple process.

How to Refer

What to expect from us

Complete Initial Assessment

We will complete the Initial Assessment at the pre-scheduled time in the client's home. Initial Assessments include a thorough review of referral, comprehensive subjective & objective assessment, positive goal setting, and, co-construction of management plan with the client.

Complete Management Plan

Approved review appointments will be scheduled to fit the client's routine. We'll keep you updated via email. Review appointments include monitoring progress, adjusting home exercise program (as appropriate), and supervision of more challenging exercises. 

Provide HCP Report

After every Initial Assessment, a comprehensive report will be forwarded to the Case Manager. The report will provide an outline of assessment findings, client goals, future recommendations, and, the proposed management plan. No further appointments will be scheduled until the plan is approved. 

Provide Progress Updates

On completion of the approved management plan, baseline outcome measures will be re-assessed to monitor client improvement. A Progress Report will be forwarded to the Case Manager. This report will include the client's current status, engagement with management plan, clinical progress, and future recommendations.
